@article{fdi:010093467, title = {{A} competing risks duration model to study {P}r{EP} discontinuation among {MSM} in {F}rance : {T}he {ERAS} 2023 study}, author = {{B}outet, {C}l{\'e}ment and {D}i {C}iaccio, {M}arion and {S}pire, {B}. and {V}elter, {A}. and {S}agaon {T}eyssier, {L}uis}, editor = {}, language = {{ENG}}, abstract = {{A}lthough {P}r{EP} prevents {HIV} among at-risk populations, high discontinuation rates hinder its effectiveness. {W}e investigated three reasons for {P}r{EP} discontinuation in real-life settings in {F}rance and associated factors, in order to assess weariness taking {P}r{EP}. {W}e used data from the {F}rench online survey {E}nqu & ecirc;te {R}apport au {S}exe 2023 which targeted gay and bisexual men having sex with men ({GBMSM}). {F}irst, we constructed the outcome '{P}r{EP} duration'. {S}econd, we created a three-category 'reason for {P}r{EP} discontinuation' variable as follows: changes in sexual behaviors, a desire to discontinue {P}r{EP}, and clinical reasons. {T}hird, we calculated {P}r{EP} survival probability. {W}e then performed a {W}eibull accelerated failure time model with competing risks to assess how the duration of time on {P}r{EP} influenced discontinuation, and whether specific factors were associated with different discontinuation reasons. {T}he analysis included 4819 {GBMSM}. {M}edian {P}r{EP} duration was 19 months. {D}iscontinuation because of changes in sexual behaviors was less likely among {GBMSM} who initiated {P}r{EP} prescription in a sexual health structure ({AF} = 0.604 [0.424-0.858]). {A} desire to stop {P}r{EP} was more likely among those living in regions with the lowest reported seropositivity rates ({AF} = 2.320 [1.054-5.107]). {D}iscontinuing {P}r{EP} for clinical reasons was more likely among participants coming from smaller municipalities ({AF} = 2.312 [1.099-4.868]). {P}r{EP} duration dependence was negative for all three reasons, implying that the longer the time taking {P}r{EP}, the lower the probability of discontinuation. {W}e found no weariness with {P}r{EP} use among {GBMSM}; this highlights the need to detail specific factors associated with {P}r{EP} discontinuation.}, keywords = {{HIV} ; {P}r{EP} ; {MSM} ; {D}uration ; {W}eibull model ; {FRANCE}}, booktitle = {}, journal = {{AIDS} and {B}ehavior}, volume = {29}, numero = {}, pages = {2711--2727}, ISSN = {1090-7165}, year = {2025}, DOI = {10.1007/s10461-025-04729-4}, URL = {https://www.documentation.ird.fr/hor/fdi:010093467}, }
