  abstract = {{T}he endothelial protein {C} receptor ({EPCR})-rs867186 {G} allele has been linked to high plasma levels of soluble {EPCR} (s{EPCR}) and controversially associated with either susceptibility or resistance to severe and cerebral malaria. {I}n this study, quantitative enzyme-linked immunosorbent assay and sequencing were used to assess s{EPCR} levels and {EPCR}-rs867186 polymorphism in blood samples from {B}eninese children with different clinical presentations of malaria. {O}ur findings show that s{EPCR} levels were higher at hospital admission than during convalescence and that {EPCR}-rs867186 {G} allele was associated with increased s{EPCR} plasma levels, malaria severity, and mortality rate ({P} < .001, {P} = .03, and {P} = .04, respectively), suggesting a role of s{EPCR} in the pathogenesis of severe malaria. {S}oluble and endothelial membrane-bound endothelial protein {C} receptor ({EPCR}) were found to be involved in cerebral malaria. {A}n {A}-to-{G} substitution at the {EPCR}-rs867186 locus is strongly associated with increased plasma-soluble {EPCR} levels, malaria severity, and mortality rate.},
