@article{fdi:010083317, title = {{E}xpansion of {KPC}-producing {E}nterobacterales in four large hospitals in {H}anoi, {V}ietnam}, author = {{L}inh, {T}. {D}. and {T}hu, {N}. {H}. and {S}hibayama, {K}. and {S}uzuki, {M}. and {Y}oshida, {L}. and {T}hai, {P}. {D}. and {A}nh, {D}. {D}. and {D}uong, {T}. {N}. and {T}rinh, {H}. {S}. and {T}hom, {V}. {P}. and {N}ga, {L}. {T}. {V}. and {P}huong, {N}. {T}. {K}. and {T}huyet, {B}. {T}. and {W}alsh, {T}. {R}. and {T}hanh, {L}. {V}. and {B}anuls, {A}nne-{L}aure and van {D}oorn, {H}. {R}. and {A}nh, {T}. {V}. and {H}oang, {T}. {H}.}, editor = {}, language = {{ENG}}, abstract = {{O}bjectives: : {T}he incidence of carbapenem resistance among nosocomial {G}ram-negative bacteria in {V}ietnam is high and increasing, including among {E}nterobacterales. {I}n this study, we assessed the presence of one of the main carbapenemase genes, bla({KPC}), among carbapenem-resistant {E}nterobacterales ({CRE}) from four large hospitals in {H}anoi, {V}ietnam, between 2010 and 2015, and described their key molecular characteristics. {M}ethods: : {KPC}-producing {E}nterobacterales were detected using conventional {PCR} and were further analysed using {S}1 nuclease pulsed-field gel electrophoresis ({S}1-{PFGE}), {S}outhern blotting and whole-genome sequencing ({WGS}) for sequence typing and genetic characterisation. {R}esults: : bla({KPC}) genes were detected in 122 (20.4%) of 599 {CRE} isolates. bla({KPC})-carrying plasmids were diverse in size. {K}lebsiella pneumoniae harbouring bla({KPC}) genes belonged to {ST}15 and {ST}11, whereas {KPC}-producing {E}scherichia coli showed more diverse sequence types including {ST}3580, {ST}448, {ST}709 and {ST}405. {G}enotypic relationships supported the hypothesis of circulation of a population of 'resident' resistant bacteria in one hospital through the years and of transmission among these hospitals via patient transfer. {WGS} results revealed co-carriage of several other antimicrobial resistance genes and three different genetic contexts of bla({KPC}-2). {A}mong these, the combination of {ISE}cp1-bla({CTX}-{M}) and {ISK}pn27-bla({KPC})-{D}elta {ISK}pn6 on the same plasmid is reported for the first time. {C}onclusion: : {W}e describe the dissemination of bla({KPC})-expressing {E}nterobacterales in four large hospitals in {H}anoi, {V}ietnam, since 2010, which may have started earlier, along with their resistance patterns, sequence types, genotypic relationship, plasmid sizes and genetic context, thereby contributing to the overall picture of the antimicrobial resistance situation in {E}nterobacterales in {V}ietnam.}, keywords = {{C}arbapenem resistance ; {KPC} ; {E}nterobacterales ; {V}ietnam ; {VIET} {NAM} ; {HANOI}}, booktitle = {}, journal = {{J}ournal of {G}lobal {A}ntimicrobial {R}esistance}, volume = {27}, numero = {}, pages = {200--211}, ISSN = {2213-7165}, year = {2021}, DOI = {10.1016/j.jgar.2021.09.007}, URL = {https://www.documentation.ird.fr/hor/fdi:010083317}, }
