@article{fdi:010082710, title = {{U}ser fees removal and community-based management of undernutrition in {B}urkina {F}aso : what effects on children's nutritional status ?}, author = {{Z}ombre, {D}. {Y}. and {D}e {A}llegri, {M}. and {R}idde, {V}al{\'e}ry and {Z}inszer, {K}.}, editor = {}, language = {{ENG}}, abstract = {{O}bjective: {T}o examine the effect of an intervention combining user fees removal with community-based management of undernutrition on the nutrition status in children under 5 years of age in {B}urkina {F}aso. {D}esign: {T}he study was a non-equivalent control group post-test-only design based on household survey data collected 4 years after the intervention onset in the intervention and comparison districts. {A}dditionally, we used propensity score weighting to achieve balance on covariates between the two districts, followed by logistic multilevel modelling. {S}etting: {T}wo health districts in the {S}ahel region. {P}articipants: {T}otally, 1116 children under 5 years of age residing in 41 intervention communities and 1305 from 51 control communities. {R}esults: {W}hen comparing children living in the intervention district to children living in a non-intervention district, we determined no differences in terms of stunting ({OR} = 1 center dot 13; 95 % {CI} 0 center dot 83, 1 center dot 54) and wasting ({OR} = 1 center dot 21; 95 % {CI} 0 center dot 90, 1 center dot 64), nor in severely wasted ({OR} = 1 center dot 27; 95 % {CI} 0 center dot 79, 2 center dot 04) and severely stunted ({OR} = 0 center dot 99; 95 % {CI} 0 center dot 76, 1 center dot 26). {H}owever, we determined that 3 % of the variance of wasting (95 % {CI} 1 center dot 25, 10 center dot 42) and 9 center dot 4 % of the variance of stunting (95 % {CI} 6 center dot 45, 13 center dot 38) were due to systematic differences between communities of residence. {T}he presence of the intervention in the communities explained 2 % of the community-level variance of stunting and 3 % of the community-level variance of wasting. {C}onclusions: {W}ith the scaling-up of the national free health policy in {A}frica, we stress the need for rigorous evaluations and the means to measure expected changes in order to better inform health interventions.}, keywords = {{U}ser fee removal ; {C}ommunity-based management of malnutrition ; {F}inancial accessibility ; {N}utritional status ; {S}tunting ; {W}asting contextual effect ; {B}urkina {F}aso ; {BURKINA} {FASO}}, booktitle = {}, journal = {{P}ublic {H}ealth {N}utrition}, volume = {24}, numero = {12}, pages = {3768--3779}, ISSN = {1475-2727}, year = {2021}, DOI = {10.1017/s1368980021000732}, URL = {https://www.documentation.ird.fr/hor/fdi:010082710}, }
