@article{fdi:010076401, title = {{L}ower urinary tract dysfunction in chronic {C}hagas disease : clinical and urodynamic presentation}, author = {{B}ey, {E}. and {P}aucara {C}ondori, {M}.{B}. and {G}aget, {O}. and {S}olano, {P}hilippe and {R}evollo, {S}. and {S}aussine, {C}. and {B}reni{\`e}re, {S}imone {F}r{\'e}d{\'e}rique}, editor = {}, language = {{ENG}}, abstract = {{P}urpose: {T}o describe and give an estimation of the prevalence of urinary disorders in chronic {C}hagas disease, since most clinical research has been centered on the description of the cardiac and digestive forms. {M}ethods: {T}o explore this topic, a cross-sectional study was conducted in 137 {B}olivian adults of both sexes suffering from symptomatic chronic {C}hagas disease. {A}ll patients presenting confirmed chagasic cardiomyopathy, megacolon or both underwent a urologic symptom questionnaire, uroflowmetry, urinary tract ultrasonography and a creatinine assay. {W}hen urinary abnormality was detected, a complete urodynamic study was proposed including cystometry, pressure-flow studies and urethral pressure profile. {R}esults: {O}ut of all study patients, 35 (26%) had a {C}hagas cardiomyopathy, 81 (59%) a megacolon, and 21 (15%) a megacolon associated with cardiomyopathy. {I}n all, 63% presented urinary disorders defined by {IPSS} > 7 and/or {ICIQ} {SF} > 1. {A}mong them, 62% were incontinent, mainly by bladder overactivity, and 45% presented grade 2 or 3 renal insufficiency. {O}f 49 patients, the urodynamic study identified 34 patients with detrusor overactivity (69%), mostly in those with {C}hagas megacolon. {M}edian bladder functional capacity, urethral closure pressure and bladder compliance had normal values. {M}oreover, 36% of these patients presented moderate hypocontractility, without significant post-void residual. {C}onclusions: {T}his study evidenced lower urinary tract dysfunction in a majority of chronic chagasic patients; those presenting megacolon were more likely to suffer from urinary incontinence. {T}hese results strongly suggest including routine urological clinical investigation in chronic {C}hagas patients, as urinary incontinence due to overactive bladder is frequently observed in this population.}, keywords = {{TRYPANOSOMIASE} {HUMAINE} ; {MALADIE} {DE} {CHAGAS} ; {ESSAI} {CLINIQUE} ; {DESORDRE} {URINAIRE} ; {BOLIVIE}}, booktitle = {{I}ncluding the topical section : progress in imaging for advanced prostate cancer}, journal = {{W}orld {J}ournal of {U}rology}, volume = {37}, numero = {7}, pages = {1395--1402}, ISSN = {0724-4983}, year = {2019}, DOI = {10.1007/s00345-018-2512-3}, URL = {https://www.documentation.ird.fr/hor/fdi:010076401}, }
