@article{fdi:010058997, title = {{B}urden of visceral leishmaniasis in villages of eastern gedaref state, {S}udan : an exhaustive cross-sectional survey}, author = {{M}ueller, {Y}. {K}. and {N}ackers, {F}. and {A}hmed, {K}. {A}. and {B}oelaert, {M}. and {D}joumessi, {J}. {C}. and {E}ltigani, {R}. and {G}orashi, {H}. {A}. and {H}ammam, {O}. and {R}itmeijer, {K}. and {S}alih, {N}. and {W}orku, {D}. and {E}tard, {J}ean-{F}ran{\c{c}}ois and {C}happuis, {F}.}, editor = {}, language = {{ENG}}, abstract = {{B}ackground: {S}ince {D}ecember 2009, {M}edecins {S}ans {F}rontieres has diagnosed and treated patients with visceral leishmaniasis ({VL}) in {T}abarak {A}llah {H}ospital, eastern {G}edaref {S}tate, one of the main endemic foci of {VL} in {S}udan. {A} survey was conducted to estimate the {VL} incidence in villages around {T}abarak {A}llah. {M}ethods: {B}etween the 5th of {M}ay and the 17th of {J}une 2011, we conducted an exhaustive door-to-door survey in 45 villages of {A}l-{G}ureisha locality. {D}eaths were investigated by verbal autopsies. {A}ll individuals with (i) fever of at least two weeks, (ii) {VL} diagnosed and treated in the previous year, and (iii) clinical suspicion of post-kala-azar dermal leishmaniasis ({PKDL}) were referred to medical teams for case ascertainment. {A} new case of {VL} was a clinical suspect with a positive rk39 rapid test or direct agglutination test ({DAT}). {R}esults: {I}n the 45 villages screened, 17,702 households were interviewed, for a population of 94,369 inhabitants. {T}he crude mortality rate over the mean recall period of 409 days was 0.13/10'000 people per day. {VL} was a possible or probable cause for 19% of all deaths. {T}he {VL}-specific mortality rate was estimated at 0.9/1000 per year. {T}he medical teams examined 551 individuals referred for a history of fever of at least two weeks. {O}ut of these, 16 were diagnosed with primary {VL}. {T}he overall incidence of {VL} over the past year was 7.0/1000 persons per year, or 7.9/1000 per year when deaths possibly or probably due to {VL} were included. {O}verall, 12.5% (11,943/95,609) of the population reported a past {VL} treatment episode. {D}iscussion and {C}onclusion: {VL} represents a significant health burden in eastern {G}edaref {S}tate. {A}ctive {VL} case detection had a very low yield in this specific setting with adequate access to care and may not be the priority intervention to enhance control in similar contexts.}, keywords = {{SOUDAN}}, booktitle = {}, journal = {{P}los {N}eglected {T}ropical {D}iseases}, volume = {6}, numero = {11}, pages = {e1872}, ISSN = {1935-2735}, year = {2012}, DOI = {10.1371/journal.pntd.0001872}, URL = {https://www.documentation.ird.fr/hor/fdi:010058997}, }
