@article{fdi:010049621, title = {{S}leeping sickness diagnosis : use of buffy coats improves the sensitivity of the mini anion exchange centrifugation test}, author = {{C}amara, {M}. and {C}amara, {O}. and {I}lboudo, {H}. and {S}akande, {H}. and {K}abor{\'e}, {J}. and {N}'{D}ri, {L}. and {J}amonneau, {V}incent and {B}ucheton, {B}runo}, editor = {}, language = {{ENG}}, abstract = {{OBJECTIVES} {T}o evaluate a modification of the mini anion exchange centrifugation test (m{AECT}) for the diagnosis of {T}rypanosoma brucei ({T}.b.) gambiense human {A}frican trypanosomiasis ({HAT}). {T}o increase its sensitivity, this test uses 350 mu l of buffy coat withdrawn from 5 ml of blood instead of blood. {METHODS} {T}he new protocol was first tested experimentally on serial dilution of trypanosomes and was then further evaluated under field conditions on 57 patients with {HAT} diagnosed during a medical survey in {G}uinea. {RESULTS} {E}xperimentally, the use of buffy coats improved m{AECT} sensitivity at least five fold and enabled to consistently detect parasites in blood at a concentration of 10 trypanosomes/ml. {D}uring the field evaluation, more patients tested positive by m{AECT}-bc (96.5%) than by m{AECT}-blood (78.9%, chi(2) = 6.93, {P} = 0.008) and lymph juice examination (77.2%, chi(2) = 7.67, {P} = 0.005). {F}urthermore, the number of parasites per collectors was significantly higher (7.2 vs. 2.6, {P} = 0.001) when buffy coats were used instead of blood. {CONCLUSION} {T}he use of the m{AECT}-bc protocol enabled a significant improvement of {HAT} parasitological diagnosis in {G}uinea, without any additional costs. {I}t would deserve to be tested in other {T}.b. gambiense endemic areas.}, keywords = {buffy coat ; diagnosis ; {H}uman {A}frican trypanosomiasis ; mini-anion ; exchange centrifugation technique (m{AECT})}, booktitle = {}, journal = {{T}ropical {M}edicine and {I}nternational {H}ealth}, volume = {15}, numero = {7}, pages = {796--799}, ISSN = {1360-2276}, year = {2010}, DOI = {10.1111/j.1365-3156.2010.02546.x}, URL = {https://www.documentation.ird.fr/hor/fdi:010049621}, }
