@article{fdi:010040714, title = {{E}fficacy of artesunate plus amodiaquine, artesunate plus sulfadoxine-pyrimethamine, and chloroquine plus sulfadoxine-pyrimethamine in patients with uncomplicated {P}lasmodium falciparum in the {C}omoros {U}nion}, author = {{T}all, {A}. and {R}abarijaona, {L}. {P}. and {R}obert, {V}incent and {B}edja, {S}. {A}. and {A}riey, {F}. and {R}andrianarivelojosia, {M}.}, editor = {}, language = {{ENG}}, abstract = {{H}ealth policy makers in {C}omoros {U}nion have considered a policy change recommending combination treatment to control malaria. {W}e evaluated the efficacy of three antimalarial drug combinations, taken orally, to enable the authorities to make an evidence-based choice. {T}he study was carried out in patients of 2-70 years old in {M}oroni, {M}oheli and {A}njouan in 2003. {W}e enrolled 168 patients with uncomplicated malaria from 1097 outpatients screened at the health centres. {O}ne hundred and fifty-eight patients, of whom half were under five years old, (mean age = 11.1 +/- 13.9 years), were followed up for 14 days. {A}ccording to {PCR} adjusted outcome, the therapeutic efficacy of artesunate + amodiaquine ({AS} + {AQ}) (n = 54) and artesunate + sulfadoxine-pyrimethamine ({AS} + {SP}) (n = 53) was 100%, whereas that of chloroquine + sulfadoxine-pyrimethamine ({CQ} + {SP}) was 98% (50/51). {T}he key difference between these treatments was the higher parasite clearance rate on {D}ay 2 obtained with artesunate-containing combinations ({P} < 0.001). {T}hese results provide a baseline for monitoring changes in the susceptibility of {P}lasmodium falciparum to artesunate+amodiaquine and artesunate + sulfadoxine-pyrimethamine ({ACT}s) in the {C}omoros {U}nion. {H}ealth policy changes involving the replacement of chloroquine in the {I}ndian {O}cean subregion are discussed.}, keywords = {malaria ; {C}omoros union ; {ACT} ; artesunate ; amodiaquine ; drug policy}, booktitle = {}, journal = {{A}cta {T}ropica}, volume = {102}, numero = {3}, pages = {176--181}, ISSN = {0001-706{X}}, year = {2007}, DOI = {10.1016/j.actatropica.2007.03.004}, URL = {https://www.documentation.ird.fr/hor/fdi:010040714}, }
