@article{fdi:010068693, title = {{K}aposi sarcoma risk in hiv-infected children and adolescents on combination antiretroviral therapy from {S}ub-{S}aharan {A}frica, {E}urope, and {A}sia}, author = {{T}anser, {F}. and {V}inikoor, {M}. and {M}acete, {E}. and {J}ourdain, {G}onzague and et al.}, editor = {}, language = {{ENG}}, abstract = {{B}ackground. {T}he burden of {K}aposi sarcoma ({KS}) in human immunodeficiency virus ({HIV})-infected children and adolescents on combination antiretroviral therapy (c{ART}) has not been compared globally. {M}ethods. {W}e analyzed cohort data from the {I}nternational {E}pidemiologic {D}atabases to {E}valuate {AIDS} and the {C}ollaboration of {O}bservational {HIV} {E}pidemiological {R}esearch in {E}urope. {W}e included {HIV}-infected children aged <16 years at c{ART} initiation from 1996 onward. {W}e used {C}ox models to calculate hazard ratios ({HR}s), adjusted for region and origin, sex, c{ART} start year, age, and {HIV}/{AIDS} stage at c{ART} initiation. {R}esults. {W}e included 24 991 children from eastern {A}frica, southern {A}frica, {E}urope and {A}sia; 26 developed {KS} after starting c{ART}. {I}ncidence rates per 100 000 person-years ({PY}s) were 86 in eastern {A}frica (95% confidence interval [{CI}], 55-133), 11 in southern {A}frica (95% {CI}, 4-35), and 81 (95% {CI}, 26-252) in children of sub-{S}aharan {A}frican ({SSA}) origin in {E}urope. {T}he {KS} incidence rates were 0/100 000 {PY}s in children of non-{SSA} origin in {E}urope (95% {CI}, 0-50) and in {A}sia (95% {CI}, 0-27). {KS} risk was lower in girls than in boys (adjusted {HR} [a{HR}], 0.3; 95% {CI}, .1-. 9) and increased with age (10-15 vs 0-4 years; a{HR}, 3.4; 95% {CI}, 1.2-10.1) and advanced {HIV}/{AIDS} stage ({CDC} stage {C} vs {A}/{B}; a{HR}, 2.4; 95% {CI},.8-7.3) at c{ART} initiation. {C}onclusions. {HIV}-infected children from {SSA} but not those from other regions, have a high risk of developing {KS} after c{ART} initiation. {E}arly c{ART} initiation in these children might reduce {KS} risk.}, keywords = {{K}aposi sarcoma ; {HIV} ; children ; antiretroviral therapy ; cohort study ; {AFRIQUE} {SUBSAHARIENNE} ; {EUROPE} ; {ASIE} ; {AFRIQUE} {DU} {SUD} ; {ZAMBIE} ; {ZIMBABWE} ; {CAMBODGE} ; {INDE} ; {INDONESIE} ; {MALAISIE} ; {THAILANDE} ; {VIETNAM} ; {AUTRICHE} ; {DANEMARK} ; {FRANCE} ; {ALLEMAGNE} ; {GRECE} ; {PAYS} {BAS} ; {ESPAGNE} ; {ROYAUME} {UNIS} ; {IRELANDE}}, booktitle = {}, journal = {{C}linical {I}nfectious {D}iseases}, volume = {63}, numero = {9}, pages = {1245--1253}, ISSN = {1058-4838}, year = {2016}, DOI = {10.1093/cid/ciw519}, URL = {https://www.documentation.ird.fr/hor/fdi:010068693}, }
