@article{fdi:010066189, title = {{R}efractory hypoglycaemia in a dog infected with {T}rypanosoma congolense}, author = {{D}eschamps, {J}. {Y}. and {D}esquesnes, {M}. and {D}orso, {L}. and {R}avel, {S}ophie and {B}ossard, {G}. and {C}harbonneau, {M}. and {G}arand, {A}. and {R}oux, {F}. {A}.}, editor = {}, language = {{ENG}}, abstract = {{A} 20 kg {G}erman shepherd dog was presented to a {F}rench veterinary teaching hospital for seizures and hyperthermia. {T}he dog had returned 1 month previously from a six-month stay in {S}enegal and sub-{S}aharan {A}frica. {B}iochemistry and haematology showed severe hypoglycaemia (0.12 g/{L}), anaemia and thrombocytopenia. {D}espite administration of large amounts of glucose (30 m{L} of 30% glucose {IV} and 10 m{L} of 70% sucrose by gavage tube hourly), 26 consecutive blood glucose measurements were below 0.25 g/{L} (except one). {R}outine cytological examination of blood smears revealed numerous free extracytoplasmic protozoa consistent with {T}rypanosoma congolense. {PCR} confirmed a {T}rypanosoma congolense forest-type infection. {T}reatment consisted of six injections of pentamidine at 48-hour intervals. {T}rypanosomes had disappeared from the blood smears four days following the first injection. {C}linical improvement was correlated with the normalization of laboratory values. {T}he infection relapsed twice and the dog was treated again; clinical signs and parasites disappeared and the dog was considered cured; however, 6 years after this incident, serological examination by {ELISA} {T}. congolense was positive. {T}he status of this dog (infected or non-infected) remains unclear. {H}ypoglycaemia was the most notable clinical feature in this case. {I}t was spectacular in its severity and in its refractory nature; glucose administration seemed only to feed the trypanosomes, indicating that treatment of hypoglycaemia may in fact have been detrimental.}, keywords = {{T}rypanosoma congolense forest-type ; {D}og ; {H}ypoglycaemia ; {F}rance ; {FRANCE}}, booktitle = {}, journal = {{P}arasite}, volume = {23}, numero = {}, pages = {art. 1 [9 p.]}, ISSN = {1252-607{X}}, year = {2016}, DOI = {10.1051/parasite/2016001}, URL = {https://www.documentation.ird.fr/hor/fdi:010066189}, }
