@article{PAR00012925, title = {{C}ryptoccocal meningitis in {Y}aounde ({C}ameroon) {HIV} infected patients : diagnosis, frequency and {C}ryptococcus neoformans isolates susceptibility study to fluconazole}, author = {{N}gouana, {T}. {K}. and {D}ongtsa, {J}. and {K}ouanfack, {C}. and {T}onfack, {C}. and {F}omena, {S}. and {M}allie, {M}. and {D}elaporte, {E}ric and {B}oyomd, {F}. {F}. and {B}ertout, {S}.}, editor = {}, language = {{ENG}}, abstract = {{C}ryptococcal meningitis is a mycosis encountered especially in patients with {A}cquired {I}mmunodeficiency {S}yndrome and is fatal in the absence of treatment. {I}nformation on epidemiology, diagnosis and susceptibility profile to antifungal drugs, are scarce in {C}ameroon. {A}uthors evaluated the diagnosis possibilities of the cryptococcal meningitis in {C}ameroon, and studied the antifungal susceptibility of isolated strains to fluconazole, used as first line treatment of the disease in {C}ameroon. {B}etween {D}ecember 2009 and {J}uly 2011,146 cerebrospinal fluids obtained from {HIV} patients with suspicion of meningitis were analysed. {T}he diagnosis procedure involved macroscopic and cyto-chemical analysis, {I}ndia ink test, culture on {S}abouraud chloramphenicol medium and antigen latex agglutination test. {A}ntifungal susceptibility testing of isolated strains to fluconazole was done by the {E}-test ({R}) method. {T}he diagnosis of cryptococcal meningitis gave 28.08% positive cases. {A}mong these patients, 80% were at stages {III} and {IV} and 20% at stage {I} of the {HIV} infection, according to the {WHO} previous classification. {C}yto-chemical analysis showed current findings in the case of cryptococcal meningitis. {I}ndia ink test and latex agglutination test exhibited very high sensitivity and specificity (> 94%). {F}luconazole antifungal susceptibility testing gave {MIC}s lower than 32 mu g/m{L} to 92.7% of isolated strains and {MIC}s greater than this value to 7.3% of isolates. {T}hese results showed that cryptococcal meningitis remains a real problem among {HIV} infected patients in {Y}aounde. {T}he emergence of fluconazole reduced susceptibility strains is worrying. {N}evertheless, efficacy of rapid detection tests is interesting because this will help in rapid diagnosis and treatment of patients.}, keywords = {{C}ryptococcosis ; {HIV} infection ; {D}iagnosis ; {F}requency ; {F}luconazole ; {CAMEROUN}}, booktitle = {}, journal = {{J}ournal de {M}ycologie {M}edicale}, volume = {25}, numero = {1}, pages = {11--16}, ISSN = {1156-5233}, year = {2015}, DOI = {10.1016/j.mycmed.2014.10.016}, URL = {https://www.documentation.ird.fr/hor/{PAR}00012925}, }
