@article{PAR00010225, title = {{Q} fever and pregnancy : disease, prevention, and strain specificity}, author = {{A}ngelakis, {E}. and {M}illion, {M}. and {D}'{A}mato, {F}. and {R}ouli, {L}. and {R}ichet, {H}. and {S}tein, {A}. and {R}olain, {J}. {M}. and {R}aoult, {D}idier}, editor = {}, language = {{ENG}}, abstract = {{T}he link between fetal morbidity and {Q} fever and the necessity of long-term antibiotics for {C}oxiella burnetii infection during pregnancy have been recently questioned in the {N}etherlands, where the clone responsible for the {Q} fever outbreak harbors the {Q}p{H}1 plasmid. {I}n this context, we assessed pregnancy outcomes according to antibiotic administration in a new series and compared the plasmid type between isolates associated with abortion and other clinical isolates to determine if there is a link between genotype and abortion in humans. {A}ll {F}rench patients who received a diagnosis of {Q} fever during pregnancy at the {F}rench {N}ational {R}eferral {C}entre for {Q} {F}ever from 2006 through {J}uly 2011 were included. {O}n the other hand, the plasmid types of 160 clinical isolates, including seven isolates from patients who experienced an abortion, were compared. {T}he differences between the {Q}p{DV} and {Q}p{H}1 plasmid sequences were analyzed. {A}cute {Q} fever was a cause of fetal morbidity, and the absence of long-term cotrimoxazole therapy was associated with fetal death (p < 0.0001). {G}enotypic analysis showed that the {Q}p{DV} plasmid was more frequent in isolates associated with abortion (p = 0.03). {A} comparison of the plasmid sequences revealed that four {Q}p{DV} proteins had no direct counterparts in {Q}p{H}1, with two whose functions were not present in {Q}p{H}1. {T}he different obstetrical morbidity of {C}. burnetii relative to different geographical areas could be related to strain specificity, possibly based on differences in plasmid sequences, or to a failure of public health authorities to detect early miscarriages.}, keywords = {{FRANCE} ; {PAYS} {BAS}}, booktitle = {}, journal = {{E}uropean {J}ournal of {C}linical {M}icrobiology and {I}nfectious {D}iseases}, volume = {32}, numero = {3}, pages = {361--368}, ISSN = {0934-9723}, year = {2013}, DOI = {10.1007/s10096-012-1750-3}, URL = {https://www.documentation.ird.fr/hor/{PAR}00010225}, }
